The clinical high risk for psychosis (CHR) paradigm represents one of the most established preventive approaches in clinical psychiatry, targeting at persons who suffer from their CHR symptoms and related impairments, and seek help for it. Specialised CHR mental health services that provide prognostic assessment and preventive treatment for CHR patients have been implemented worldwide and, until recently, have been working mainly on an individual basis and within research programmes not covered by health insurances. Yet, in recent years, national programs to translate the scientific evidence into clinical practice have been increasingly set up, thereby facing different challenges and advantages.
National programmes from four different countries will be presented to gain insight into the possibilities and challenges of implementing CHR services on a national level and within the framework of different health care systems. Prof. Armando will report on a CHR network in Switzerland that has a universal health service with compulsory closing of a private health insurance. Prof. Laprévote will present a research network in France funded by INSERM, which aims to implement research based on clinical staging in expert and early intervention centres. Dr. Mayol will introduce a national CHR network from Chile that operates under a unique mixed public-private health care system. Finally, Prof. Meisenzahl will introduce a large national programme to test the implementation of artificial intelligence-assisted prevention of psychoses in Germany that runs universal multi-payer health care system paid for by a combination of statutory and private health insurance.
