Regular physical activity (PA) is a cornerstone of good health in the ageing population. Cardiorespiratory fitness is an important surrogate marker for survival. In primary cardiovascular disease prevention, PA is considered an effective monotherapy with beneficial effects on the cardiovascular risk profile (e.g. lipid profile, blood pressure, diabetes mellitus, body composition) and endothelial function. In secondary prevention, PA is an addition to, rather than a replacement for evidence-based medication. There is growing evidence that high intensity interval training (HIIT) has superior effects on relevant clinical endpoints (cardiorespiratory fitness, cardiac remodeling, quality of life) compared to the established continuous moderate intensity training. Prevalence of coronary artery disease increases with age. Therefore the risk for exercise-induced sudden cardiac death should be taken into account in middle-aged/senior athletes. Determination of the cardiovascular risk profile and according lifestyle-modifications are crucial in the aging non-athlete population. While exercise testing is used for detection of significant coronary stenosis in the clinical setting, it does not reliably predict the risk of plaque rupture.
