The current approach for the diagnosis of aplastic anemia is based on morphology and cytogenetics of hematopoietic cells. The morphological characteristics and similarities with other bone marrow failures disorders represent a real challenge in the differential diagnosis. The evolving better understanding of the subjacent mechanisms involved in bone marrow failures has made a major contribution in the field, however clear delimitation between the different entities need still improvement. New advances in diagnostic technology provide permanently additional information, which might become the basis to solve unmet diagnostic problems. This paper reviews the current approach for the diagnosis of acquired aplastic anemia in adults and describes recent progress in the field with potential implications for diagnosis and treatment.
