We report a 79-year-old female patient with recurrent episodes of presyncope/syncope over 15 years, accompanied by urge incontinence and nocturnal enuresis, referred to the psychosomatic ward, as her medical symptoms were unexplained. Based on a lack of neurological deficits, extensive clinical examinations without any diagnostic cues, a typical history, orthostatic challenge and plasma catecholamine findings, we were able to diagnose pure autonomic failure. We could exclude psychosocial stress or a mental disorder as relevant causes for the symptoms.
