(Request a copy)Pneumonia continues to be one of the most important infectious diseases which often leads to hospital admissions and is occasionally fatal. The spectrum of causative organisms, their sensitivity pattern to antibiotics, diagnostic tools, and available antibiotics are continually changing. Currently, the most disquieting trend is the increasing development of resistance to commonly used antibiotics by the pneumococcus. Although this trend has thus far been observed primarily in other countries, it will most likely not spare Switzerland. Rational empiric therapy must include careful clinical assessment of the patient, knowledge of the spectrum of organisms locally causing pneumonias, including their resistance patterns, as well as a prognostic assessment of the patient. Using these factors, possible antibiotic schemes for empiric therapy of community-acquired pneumonia are reviewed.
